Meet Our Clinical Care Coordinators

Meet Kim and Jill, our in-house Clinical Care Coordinators! They’re Registered Nurses who ensure the care provided to our participants always meets the highest and most current standards.  

They’re on hand to assess our participants’ needs, develop and review individualised care plans, and provide up-to-date training to support workers.  

Plus, they’re both genuinely lovely people, who enjoy seeing our participants thrive! 

What does a Clinical Coordinator do? 

Kim and Jill make house visits to each of their clients at least once a month, checking in on their wellbeing, reviewing their care plans and medication records, and attending to any clinical concerns they may have. They:

  • develop individualised care plans, which they consistently review to ensure the client is receiving the best standard of care for their needs. 
  • collaborate with Allied Health providers. They’re able to help explain and advocate for their participants, bringing a clinical perspective when speaking to GPs and other health care providers. 
  • can provide clinical recommendations for participants who need a higher ratio of care than their NDIS plan currently funds. 
  • ensure each support worker is properly trained to care for their participant, in areas such as hoisting, PEG feeding, diabetes management, seizure management and dementia. They liaise with our Service Coordinators to confirm that each worker on the roster has the appropriate training. 

Connection and Care 

Before earning nursing qualifications, Kim was a support worker herself.  

“I know the day-to-day job (of providing support), but in this role I’m involved in the bigger picture of that person’s care. I can work with their GPs, specialists, and with Allied Health. This is a vulnerable population we work with, so we protect them – like they’re our own family,” she says. 

Jill brings a different perspective, coming to the role directly from a hospital nursing setting, and she’s enjoying the community aspect of this role. 

“I like the relationship-building. I can get to know the person’s base line, so I can see when they seem even a little bit unwell; we can take observations, investigate, and book appointments for them,” she explains. 

Kim adds, “For the participant, and for their loved ones, we can ease their worries – it’s peace of mind for them, knowing we have that clinical perspective. They’re in good hands with us.”
